Sedana Medical Research Grant (SMRG), som inr\u00e4ttades 2019, \u00e4r en integrerad del av detta uppdrag. Prioritet kommer att ges till forskningsprojekt som unders\u00f6ker effekterna av inhalerad isofluran som levereras via Sedaconda ACD till mekaniskt ventilerade patienter. V\u00e4lformulerade ans\u00f6kningar om andra forsknings\u00e4mnen relaterade till inhalationssedering kan dock ocks\u00e5 komma att beaktas. Ytterligare resultat kommer att utv\u00e4rdera farmakokinetiken f\u00f6r inhalation av lugnande medel som en funktion av minutventilation, d\u00f6dutrymmesventilation, CO22-eliminering och lungvolym. <\/p>\n<p>Studien kommer att omfatta vuxna patienter med ARDS som f\u00e5r invasiv PSV och sederas med IV analgo-sedering och som f\u00f6rv\u00e4ntas forts\u00e4tta med PSV med en stabil Richmond Agitation and Sedation Scale (RASS) i 24 timmar eller l\u00e4ngre. Under 6 timmar kommer patienterna att f\u00e5 inhalationssedering som titreras till en likv\u00e4rdig RASS och kommer att j\u00e4mf\u00f6ras, i en cross-over studiedesign, med standardbehandling med IV-sedering (propofol och fentanyl). Inhalationssedering kan leda till en h\u00f6gre grad av spontanandning. Prekliniska studier av inhalationssedering vid experimentell ARDS har visat en minskning av lung\u00f6dem, men detta har inte studerats p\u00e5 m\u00e4nniskor. <\/p>\n<p>I denna studie antas att mekanismen bakom sevofluranmedierad f\u00f6rb\u00e4ttrad syres\u00e4ttning vid ARDS \u00e4r minskat lung\u00f6dem.<\/p>\n<p>Detta forskningsprojekt kommer att utv\u00e4rdera effekterna av inhalationssedering via AnaConDa, j\u00e4mf\u00f6rt med propofol, p\u00e5 extravaskul\u00e4rt lungvattenindex (EVLWi) och pulmonellt vaskul\u00e4rt permeabilitetsindex (PVPI) hos ARDS-patienter. P\u00e5 grund av den stigande \u00e5ldern hos patienter som genomg\u00e5r komplex hj\u00e4rtkirurgi, med en \u00f6kande f\u00f6rekomst av lever- och njurdysfunktion, \u00e4r metabolismen av intraven\u00f6sa lugnande medel delvis allvarligt nedsatt och kan leda till \u00f6versedering, f\u00f6rl\u00e4ngd mekanisk ventilation, delirium och l\u00e4kemedelsinducerad hypotoni. <\/p>\n<p>Allt fler data tyder p\u00e5 att flyktig sedering leder till att patienterna vaknar snabbare och att extuberingstiderna f\u00f6rkortas inom kirurgisk intensivv\u00e5rd. Specifikt skiljer sig (multipel) klaffkirurgi som kr\u00e4ver l\u00e5ngvarig intervention och tid p\u00e5 kardiopulmonell bypass fr\u00e5n CABG-kirurgi. Administrering f\u00f6re en l\u00e5ngvarig ischemisk episod i hj\u00e4rtat kallas anestesisk prekonditionering. <\/p>\n<p>Flera kliniska studier tyder p\u00e5 att flyktiga anestetika som ges under hj\u00e4rtkirurgi ger myokardskydd och minskar risken f\u00f6r perioperativ hj\u00e4rtinfarkt, myokarddysfunktion och d\u00f6d.
